There are a variety of different diabetes education programmes within Bradford.

If you have been diagnosed with Type 2 Diabetes and would like us to refer you to one of these programmes, Please complete this form.

We can create you an account on MapMyDiabetes as well as referring you to one of the face-to-face programmes if you would like.


MapMyDiabetes is an online education and support system for people with type 2 diabetes. It can be used on your computer, laptop, tablet, or smartphone.

MapMyDiabetes lets you:

  • Set goals to improve your health
  • Record your own information
  • Keep track of your progress
  • Share your information with your GP and Nurse

DiCE (Diabetes in Control Everyday)

DiCE run a variety of structured education programmes for people with type 2 diabetes.

Comprehensive type 2 diabetes courses: 2.5 hours x 6 sessions

DiCE Asian Delivered in a South Asian language: How to manage diabetes tailored to Asian culture and tradition
One-off diabetes sessions: 2 hours x 1 session
